Clarify each person’s role
A patient may receive support from several clinicians, home health nurses, facility staff, and family caregivers. Ask who provides day-to-day dressing care, who coordinates supplies, and who should be contacted about a change in the wound.
Make instructions understandable
Ask the treating team to explain the dressing plan, wound protection, pressure relief when relevant, and the follow-up arrangement. If an instruction is unclear or difficult to carry out, tell the team so the plan can be reviewed.
Follow the individualized instructions you have received rather than changing treatment based on general website information.
Keep the conversation connected
Bring concerns about pain, comfort, mobility, daily support, or difficulty following the plan to the clinician’s attention. Let the team know about changes in other medical care that may affect the wound.
Questions worth asking
- What is the purpose of each part of the treatment plan?
- Who should I call with a question between visits?
- Which changes should be reported promptly?
- How will we decide whether the plan needs to change?
