Before the visit

Our team reviews the patient’s location, care setting, referral information, and insurance details. We explain which clinical records are needed and coordinate the visit with the patient or the person arranging care.

It can help to have a current medication list, relevant wound history, existing treatment orders, and the names of the clinicians involved. Do not stop prescribed treatment while waiting for an assessment unless your treating clinician directs you to do so.

During the clinical assessment

The clinician reviews the wound and relevant medical history. The examination may consider location, dimensions, tissue, drainage, surrounding skin, discomfort, and factors that can affect care. Further evaluation is discussed when indicated.

Understanding the recommendation

Ask what the assessment suggests about the wound’s cause, the goals of care, treatment options, and what daily care should involve. Any proposed procedure should have a clear purpose, with discussion of relevant risks and alternatives.

Leave with a clear understanding

Before the visit ends, ask the clinician to explain the plan in terms you can use at home or share with the care team.

  • What is the purpose of the recommended treatment?
  • Who will provide care between clinical visits?
  • Which changes should be reported, and whom should we contact?
  • How and when will the wound be reassessed?

If instructions are unclear or difficult to follow, let the team know. Follow-up reflects the wound findings and the patient’s circumstances.
