Care in the setting that matters

Clinical attention.
Connected to daily care.

Our role is to support the wound care plan within the patient’s existing care arrangements, with attention to clinical needs, daily nursing responsibilities, and goals of care.

Skilled nursing facilities

Resident assessment and follow-up in coordination with attending clinicians and nursing staff. We consider the wound, current orders, clinical needs, and goals of care.

Assisted living communities

Mobile assessment at the resident’s location, with communication around treatment recommendations and the daily support needed between visits.

Home health agencies

Clinical wound care alongside existing home health services. Recommendations and responsibilities are clarified with the team providing ongoing nursing care.

Physicians & discharge teams

A clear point of contact for referral review, relevant clinical records, and arranging the next step after a transition in care.

From referral to follow-up

A practical pathway
for your team.

01

Start the conversation

Share the care setting, patient location, and the clinical need. Identify the staff member coordinating the referral.

02

Review the relevant records

Our team explains the clinical documentation, current treatment information, and insurance details needed for review.

03

Coordinate ongoing care

Arrange the assessment and the point of contact for recommendations. Follow-up reflects the wound findings and care plan.

A practical clinical handoff

A plan the whole
care team can follow.

Effective follow-through depends on clear recommendations and defined responsibilities. These are the details that connect a clinical visit with care between visits.

  • Assessment findings

The wound’s condition, relevant contributing factors, and the clinical reasoning behind the recommendations.

  • Treatment recommendations

The purpose of the dressing plan and any proposed procedure, with attention to the patient’s clinical needs and goals.

  • Care between visits

Who provides daily care, how instructions fit existing orders, and which concerns should be raised with the treating team.

  • Follow-up and reassessment

How wound changes and treatment response will be reviewed, including when further evaluation may be appropriate.

Practical questions

Built around
the existing care team.

Does Amniotti replace daily nursing care?

Our clinical visits complement the facility’s or home health agency’s ongoing nursing care. The scope and frequency of visits depend on the assessment and care plan.

Can you coordinate care for a patient receiving hospice?

When appropriate, wound care is coordinated with the hospice team and the patient’s goals. Comfort, treatment burden, care arrangements, and coverage are reviewed with that team.

What affects coverage in a facility?

The resident’s benefits, current setting, medical necessity, and services all matter. Skilled nursing, home health, and hospice arrangements can have different requirements. Our team reviews the applicable details before care is arranged.
